Applications for sewer connection permits shall be made in writing on a form prescribed by the plumbing inspector of said city and shall give the location of the property, the number of buildings or dwelling units to be connected, the name of the owner, the address of the owner, the name of the person or firm engaged to make the connection, and such applications shall state whether or not the real property to which the sewer connection is to be made has ever in the past been assessed for the construction of a sewer, and such other information or plans as may be required by the sewer inspector.
