The application for a license to operate or engage in a jukebox or commercial amusement device business in the city of North Bend shall contain the name, age and date of birth of the applicant, whether or not the applicant or its principal owners or officers has been convicted of a felony or gambling offense, and the principal place of business and the addresses of all offices of the applicant. All applications for licenses for individual machines as provided in this chapter shall contain the name and address of the applicant, the place where the machine or device is to be displayed or operated, and the type of business conducted at that place, and a description of the machine to be covered by the license, mechanical features, name of manufacturer, and serial number, and in the event that the owner of such machine is not licensed to engage in the business of distributing such machines, then such application shall also contain the information required for the business license.
